Overview The Program Manager oversees implementation of CalAIM services for the Homeless Services Department encompassing Housing and Community Supports and Enhanced Care Management. The Program Manager will track implementation components including, but not limited to, referrals, client enrollment/disenrollment, capacity, services model and workflows, revenue tracking, quality assurance, and overall programmatic and financial sustainability. Supports implementation and possible expansion of CalAIM services to new sites using best practices. Collaborates closely with site-based CalAIM staff and their managers as well as other agency CalAIM managers. This is a full time, exempt, benefit eligible position in Oakland, CA. LifeLong Medical Care is a large, multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community.
