At Mass General Brigham (MGB), we are developing an integrated healthcare system for the future. MGB, the largest healthcare system in the state of Massachusetts, has 2 academic hospitals, 3 specialty hospitals, 7 community hospitals, a health insurance provider (MGB Health Plan), and a Home Care division. The West Health Accelerator is a care model redesign program that seeks to improve care for hospitalized patients within MGB while creating a replicable model for scaling across small community hospitals to regional medical centers through large academic health systems.
The MGB Community Director of Geriatric Operations, in partnership with program leadership, is responsible for the direction and alignment of activities undertaken by the West Health Accelerator (WHA) team and, in collaboration with site leaders, will lead nursing implementation at MGB community hospitals. The West Health Accelerator Program will expand the Superior Treatment of Elders Pathway (STEP) from the Center for Geriatric Surgery (CGS) at Brigham and Women's Hospital to nine hospitals across Mass General Brigham (MGB) and package the learnings into a scalable national model. The MGB Community Director of Geriatric Operations will participate in the development, refinement, communication, and actualization of the WHA programs specific to the care of older adults and other adults with frailty across MGB community hospitals, divisions, service lines, disciplines, and role types. Leadership and Collaboration * Participate in formulating the vision and strategic plan of geriatric efforts within the West Health Accelerator * Communicate strategic vision to respective nurse leaders across participating MGB entities * Provide leadership and subject matter expertise to hospital programs that include, but are not specific to, the care of older adults * Provide input into development and implementation of information systems to support optimal patient care * Collaborate with interprofessional providers to ensure patient care is coordinated across the continuum Operational and Clinical * Oversee the development, implementation, and coordination of clinical pathways, guidelines, and protocols * Monitor and evaluate quality of geriatric care in collaboration with leaders and departments; and collaborate with site leaders to improve operations and clinical care when needed * Apply advanced knowledge and skills to prevent or solve complex and/or recurring issues Program Development and Coordination * In collaboration with MGB and WHA geriatric leadership, develop and maintain a comprehensive, effective geriatric care program that aligns with geriatric best practices and meets external standards including that of state and national programs * Act as a liaison and facilitate communication across system hospitals and departments * Collaborate with site leadership, champions, and frontline staff to support program implementation * Participate in professional groups and committees within MGB and the healthcare community to address issues related to the care of older adults Quality of Care/Performance Improvement * Apply evidence-based standards for professional practice designed to facilitate high quality patient care * Monitor the clinical processes and outcomes and system issues related to the quality of care provided to older adult patients * Collaborate on the development and reporting of quality filters, audits, chart abstraction, data collection, case reviews, data analysis Qualifications
