HealthPartners is currently hiring for an RN - Hospice Referral Facilitation Coordinator. The coordinator is responsible for ensuring continuity in the planning and coordinating of hospice & palliative care referrals which enables the appropriate review of all complexities surrounding the delivery of care and service to patients and families. The coordinator supports the referral process by researching and implementing processes that address the intricacies of highly complex end of life care and the associated cost and reimbursement of such care.

Another component of the role will include managing staffing and scheduling for hospice patients. This role involves collaborating with the nursing team to plan for patient visit coverage, scheduling admissions, and assigning visits strategically based on patient needs.

The Referral Facilitation Coordinator is directly responsible for ensuring the continuity of referral information and its' smooth and seamless transition to scheduling. The ideal candidate will be proficient in Outlook, SharePoint, Excel, and Epic EMR, using these tools to coordinate efficient and effective visit schedules while maintaining optimal patient care. Effective communication, organizational skills, and a comfort with technology are essential for success in this position.

Required Qualifications:



  • Associate's degree in nursing
  • Current RN licensure in Minnesota
  • Two (2) years of hospice or palliative care experience or related field
  • Demonstrated ability to professionally and compassionately communicate over the phone to a variety of customers.
  • Excellent written and oral communication skills
  • Ability to work independently.
  • Ability to effective work with change
  • Highly organized and detail oriented
  • Basic computer skills
  • Basic knowledge of reimbursement and regulations


Preferred Qualifications:



  • Bachelor's degree in nursing
  • 2 years hospice experience


Hours/Location:



  • M-F; Days
  • The coordinator will initially work onsite for onboarding and training. After successfully completing training, the coordinator will work in a hybrid capacity, attending onsite meetings and workgroup sessions as needed.


Responsibilities:



  • Manages patient care by coordinating referrals, assessing appropriateness and resources available to meet patient needs.
  • Shares information with members of the interdisciplinary team involved.
  • Promotes and demonstrates high quality patient care and assures compliance with documentation and clinical records.
  • Serves as a resource to staff regarding clinical, reimbursement, and regulatory complexities of patient/family care.
  • Adheres to policies and procedures in compliance with the organization, and state and federal legislation.
  • Uses interdisciplinary resources as needed to enhance clinical decision making.
  • Serves as resource for the Intake and Staffing departments, and as a liaison between physicians, hospitals, clinics, facilities, vendors, and the hospice & palliative care staff.
  • Assists in facilitating quality improvement initiatives surrounding state and federal regulations and patient/family care.
  • Assures the smooth transition of the referral from the point of contact through to the admission.
  • Uses appropriate electronic systems automated or manual to obtain patient information.
  • Reviews and makes the determination on claims as related to or unrelated to the terminal prognosis.
  • Provides patient/family informed consent over the phones as necessary, obtaining signed consents prior to the admission.
  • Serves as role model in professional conduct.
  • Tracks and provides regular follow up on all consultation visits.
  • Other duties as assigned.
