BASIC PURPOSE

The purpose of the ECM Program Manager is to manage the outreach, engagement, implementation and evaluation of Enhanced Care Management services.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  1. Provide direct supervision of Enhanced Care Management staff including but not limited scheduling and evaluating employees
  2. Ensure compliance of Enhanced Care Management services according to internal policies and contractual terms
  3. Manage Enhanced Care Management outreach and engagement activities.
  4. Manage assigned cases actively receiving Enhanced Care Management services
  5. Manage case management and service coordination workflows
  6. Transport participants for service navigation and medical appointments
  7. Advocate for assigned participants to respective outside agencies
  8. Organize and facilitate meetings with key stakeholders involved in the execution of care coordination/case management programs
  9. Actively participate in all collaborative partnerships as assigned by supervisor
  10. Manage medical referrals in coordination with contracted managed care plans
  11. Participate in weekly case conference meetings
  12. Participate in program and agency trainings as assigned
  13. Maintain accurate participant records in various information management systems; and generate reports as requested
  14. Maintain and execute confidential information according to HIPPA standards
  15. Maintain a highly detailed and organized filling system
  16. Ensure intake procedures utilize trauma informed care, harm reduction and housing first principles
  17. Check and respond to emails and voicemails on a regular basis
  18. Adhere to confidentiality standards
  19. Other duties as assigned



KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS



  1. Ability to speak and write the English language at a high and professional level
  2. High degree of confidentiality
  3. Able to endorse and promote The Salvation Army's mission
  4. Minimum three (3) years of social service/work experience preferred
  5. Basic computer skills required; electronic medical record (EMR) experience preferred
  6. Computer literate, in Windows environment (Microsoft Office), Publisher, Access and Outlook preferred
  7. Excellent communication skills, both written and verbal.
  8. Excellent and professional telephone etiquette and presence
  9. Excellent organizational skills
  10. Strong ability to utilize a high level of time management and handling multiple tasks



CERTIFICATES, LICENSES, REGISTRATIONS



  1. Bachelor's degree in Social Work, Business Administration, Human Services, Management or minimum of 3-4 year's experience in social work, homeless service management, human resource management, healthcare management/administration or related field
  2. Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
  3. Must be 21 years or older
  4. Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
  5. Complete The Salvation Army vehicle course training



PHYSICAL REQUIREMENTS:



  1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
  2. Ability to grasp, push, and/or pull objects
  3. Ability to reach overhead
  4. Ability to operate telephone



  1. Ability to lift up to 25-40 lbs.

  2. Ability to process written, visual, and/or verbal information
  3. Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.
