Summary:
The Community Health Worker (CHW) will work with patients to address social determinants of health and medical coordination needs with a focus on patients diagnosed with hypertension. The CHW will serve as a liaison between the patient's healthcare team and the community, provide short term intensive case management, and in-person support as needed to advocate for appropriate services, benefits, and programs.
Social Determinants of Health (SDOH):
*Support community resource finding and navigation related to Social Determinants of Health (SDOH) needs, including housing, food, transportation, utilities, medication costs, elder care, childcare, job search, and education needs.
*Accompaniment to community, state, and federal agencies to provide support and advocacy.
*Conduct in-person outreaches that include home visits or other identified location in the community.
*Work in partnership with patient to understand and navigate resources independently (e.g., organizing their records, manage waitlist for resources, recertifications, etc.).
*Refer to internal or external care management services when additional capacity is required to meet other social care needs (i.e., nutrition, housing, domestic violence, etc.).
Medical Coordination:
*Work in partnership with the patient and patient care team (e.g., pharmacy, specialist, PCP, medical interpreters, social workers, nurses, etc.) to identify and address barriers to care, set goals for patient's care and motivate patients to follow through.
*Accompaniment to medical appointments when needed to provide support and advocacy.
Hypertension Support:
*Educate patients on managing their hypertension, its causes and importance of medication adherence, lifestyle modifications (such as diet and exercise), and how to monitor blood pressure.
*Provide patient with blood pressure cuff, monitor and training.
*Collect blood pressure readings and facilitate communication between patient and medical providers.
*Weekly or bi-weekly follow-up services to ensure that patients adhere to their treatment plans and address barriers to care.
Administrative Duties:
*Follow documentation guidelines and protocol within health record database.
*Track SDOH and medical needs progress and completion.
*Maintain calendar of program events, activities, meetings, and in-person patient visits
*Contribute to mid-year and end of the year reports describing program and case examples.
*Attend initial and continuing education training programs including self-directed reading and in-person and online learning.

Qualifications:

The Community Health Worker position requires the ability to be self-directing, motivated, outgoing, professional, goal-oriented, organized and to work as a patient advocate with patients from diverse cultures and backgrounds.

  • Minimum 2-4 years of case management or relevant work experience in the community and/or associate degree in relevant field (e.g., Social Work, Human Services)
  • High school degree required.

  • Knowledge of community-based program services, public benefits, and other resources.
  • Ability to connect with people from all walks of life with empathy and humility.
  • Excellent oral and written communication skills.
  • Strong attention to detail and prioritization.
  • Ability to quickly adapt to changes in a fast-paced environment and competing priorities
  • Excellent organizational skills given coordination of multidisciplinary team, various logistical components, data tracking.
  • Familiarity and willingness to learn service approach models (e.g., trauma informed, cultural humility, strength-based, and others).
  • Strong time management and planning skills.
  • Ability to make connections across internal and external teams of care.
  • Ability to work well within a team and independently
  • Basic computer skills, including Microsoft Office and database management.
  • Fluency in Spanish required.
