Full-time contract for primary care/acute care APP (Evening Hours)
that is self-motivated, energetic, and a take-charge individual.
Job Responsibilities: Provides limited care coordination and acute care (i.e. laceration repair, point of care testing, asthma management, immunization updates, simple splinting, etc.) to clinic patients with acute and chronic health conditions in collaboration with other medical providers. Clinic patients include newborn to adult. Responsible for the patient evaluation, interpretation, diagnosis, and prescribing of appropriate medical and therapeutic measures. Works autonomously within a clinic comprised of physicians, APP, CNM, medical assistants, RN, pharmacists, limited specialty services, and therapists. 40% or more of this job's practice is based within an acute care/walk-in setting. 3-4 evenings per week (4-8:30) required. May also work primary care clinics with a primary care team during other shifts. Seeking candidates for a full-time contract (FTE .67-.89), which includes a complete benefits package. Spanish speaking preferred. Evaluates, diagnoses, treats, and manages acute patients within an acute care/ walk-in setting for the majority of the work shift. Acts as the primary patient contact. Provides for continuity of care by ensuring smooth transitions between care settings and tracks patients along the care continuum including post-discharge plans. Provides clinical direction to nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure the patient receives recommended care. Prescribes, recommends, and regulates pharmacologic and non-pharmacologic therapies, including monitoring for previous controlled medication use and pain contracts with limited prescribing of narcotic medications. Provides care coordination, which includes managing a variety of ancillary services, internal or external referrals, and communication with school for the school-based clinic patients. Orders, performs, and interprets procedures and diagnostic tests or studies, in the field of clinical specialty trained and within advanced practice provider scope and licensure, to manage simple conditions such as those produced by infection or trauma, as well as to assist in the management of more complex illness and injury. Examples could include: injections, immunizations, suturing, wound care, and biopsies. Counsels, instructs, and educates patients on physical and mental health including: diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development. Interprets and evaluates patient data within advanced practice provider scope and licensure for the purpose of determining management and treatment of patients. Consults with physician as appropriate. Adjusts patient care plan by reviewing and monitoring treatments and therapy plans. Prepares and signs patient care plans, encounter notes and summaries of patient care. Maintains records according to established standards and guidelines. Scheduled Hours FTE .7 - 1.0 (negotiable) Set number of 4-hour shifts per week, scheduled based on department needs. The schedule template will be predictable but with flexibility highly valued. The schedule will include at least three 'evening' shifts, 4-8pm. Clinic hours are Mon-Fri, 8am-8pm. Shifts are scheduled as 800-1200, 1200-1600, and/or 1600-2000. Benefits included (CME, PTO, health/dental, 401k) Student Loan reimbursement is available Licenses & Certifications Physician Assistant or Nurse Practitioner license issued by the state in which the team member practices, and Family Nurse Practitioner certification issued by the American Academy of Nurse Practitioner Certification Board (AANPCB) or Physician Assistant (PA-C) issued by the National Commission on Certification of Physician Assistants (NCCPA), and Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and Basic Life Support (BLS) issued by the American Heart Association (AHA) needs to be obtained within 6 months unless the department leader has determined it is not required. Degrees Master's Degree in Physician Assistant Studies (may consider Bachelor's degree with 7+years relevant experience) or Master's/Doctorate Nursing Degree in the specialty of Family Practice/Primary Care. Required Functional Experience Typically requires 2 years of experience in providing and coordinating care for patients with acute and chronic health conditions as a physician assistant or nurse practitioner. |
