The Clinical Pharmacy Supervisor provides leadership and supervision of daily operations to the Home Infusion Clinical Pharmacy Team. Assists with mentoring clinical pharmacy team employees, conducts special projects and will be assigned key business and patient management functions related to safety, business process efficiencies, medication and policy standards, quality assurance and process improvement. Assists with hiring, termination, evaluation and scheduling of assigned personnel within area.

PRIMARY RESPONSIBILITIES:

  • Reviewing and evaluating prescription orders for complex medications

  • Providing medication therapy management services

  • Dispensing and fulfillment

  • Counseling patients and caregivers

  • Collaboration with nursing and other disciplines

  • Maintaining clinical documentation

  • Ongoing patient assessment

  • Conducting interventions to minimize therapy related adverse events & problems

  • Improving adherence and clinical outcomes

  • Coordination of care activities

  • Patient care planning and discharge

  • This role will also have on call pharmacist responsibilities

ESSENTIAL FUNCTIONS:

  • Acts as a liaison between the Clinical Pharmacy Team (Clinical Pharmacists, Clinical Pharmacy Technicians and Dietitians) and the Clinical Pharmacy Manager to ensure that all patient care and business functions are optimized for effective functioning of the pharmacy.

  • Assumes additional clinical and operational responsibilities above and beyond the role of a pharmacist, which include but are not limited to new program development and enhancement, process improvement, inventory management, accreditation and regulatory compliance.

  • Reviews and supervises the work completed by clinical pharmacy staff engaged in all areas of the prescription and patient management processes including assessment, documentation, processing, labeling, packaging of pharmaceuticals and supplies.

  • Oversees mentorship activities of new and existing personnel

  • Oversees production work flows and monitors queues for bottlenecks and other challenges.

  • Develops training programs & policies and procedures.

  • Interacts with co-workers, visitors, students and other staff consistent with the values of Jefferson.

  • Other duties as assigned.

EDUCATIONAL/TRAINING REQUIREMENTS:

  • PharmD Degree or Bachelor of Science Degree in Pharmacy with 5+ years of pharmacy experience

CERTIFICATES, LICENSES, AND REGISTRATION:

  • Valid Pharmacist license in the state of Pennsylvania.

EXPERIENCE REQUIREMENTS:

  • Prior home infusion pharmacy experience or residency as a staff pharmacist, lead or higher.

  • Proven abilities in assuming and conducting responsibilities beyond the role of a patient care pharmacist.

  • Business operations experience a plus.

ADDITIONAL INFORMATION:

  • Ability to communicate clearly and effectively, verbally and in writing, with physicians/medical staff, patients, insurance companies and other staff.

  • Strong organization skills, with ability to multitask, collate data from many sources and manage several tasks at any one time.

  • Excellent problem-solving skills and the ability to adapt to the changing needs of the organization.

  • Strong business sense and acumen

  • Demonstrates a strong work ethic

  • Displays leadership skills

  • Proficiency in Microsoft Office products including Excel, Word, Power Point
