Transitional Care Programs (TCP) can transform dialysis care by specializing in new patient education, enabling patients to make an active and informed modality choice. This position teaches and educates patients and teammates on ESRD modality options including lifestyle benefits, challenges, techniques and alternatives. The educator is responsible for understanding the patient's values and adjusting the education to meet the needs of the patient, following our standard educational materials.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Educate and inform patients, patient families, and teammates about treatment options and overall dialysis education, utilizing standard materials; provide assistance with dialysis transitions as required; work with on-site interdisciplinary team to identify and educate potential and existing patients on specific modality options.
  • Coordinate and communicate with the clinic interdisciplinary team including but not limited to the in-centers nurses, social worker, dietitian and Home nurses to plan education delivery and transition steps, when appropriate, and to complete the education process
  • Monitor, track, trend, and report key metrics including but not limited to completion date of training modules, patient modality choice at end of education, and key drivers in decision. Track and report patients contacted and individual patient statistics, as applicable
  • Perform other duties and responsibilities as assigned including but not limited to attend team meetings, phone conferences, and training as needed to know, understand, and follow teammate guidelines, employment policies, and department or company procedures

EDUCATION AND EXPERIENCE

  • 2-4 years Dialysis Experience is preferred
  • High School Diploma
  • Vocational/ Technical School Certificate Degree
  • Associate's Degree or Apprenticeship

SPECIALIZED EXPERIENCE, EDUCATION, TRAINING, OR QUALIFICATIONS

  • Demonstrated ability to adapt messaging and education approach to the individual patient's circumstances and learning styles
  • Experience in teaching the adult learner with demonstrated ability to recognize and customize education to individual learning styles.
  • Experience in project management to coordinate the patient experience with teammates while systemically tracking patient progress through the education process
  • Ability to cultivate connections with patients, clinic teammates and physicians
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required; functional proficiency with DaVita applications including but not limited to SNAPPY, Reggie, Laboratory, required within 90 days of employment or position change
  • Bilingual - Preferred
  • Minimum of one (1) year's nephrology experience, home dialysis experience
  • Demonstrated knowledge of chronic, peritoneal, nocturnal, and home hemodialysis
  • Experience in case management, home health, disease management, or hospice
  • Minimum of one (1) year experience educating patients on ESRD and CKD programs

CERTIFICATES, LICENSES, AND REGISTRATIONS

  • Clinical license or certification; dialysis specific if possible (state specific)

TRAVEL AND TIME REQUIREMENTS

  • Travel: Geography dependent, but could be 75%+ (all local, clinic to clinic travel); Responsible for educating patients at 3- 10+ clinics Nights/Weekends: limited, but occasional; hours are generally 8-5 depending on patient schedules
