Responsibilities

  • Conducts in-home and telehealth assessments for patients with complex needs.
  • Prescribes new medications and performs medication reconciliation.
  • Demonstrates sound clinical judgement and prescribing proficiency in all aspects of symptom management and end of life care.
  • Facilitates family meetings, goals of care conversations, and ACP discussions.
  • Leads oversight of care plans in conjunction with the rest of the clinical care team.
  • Advocates for the patient and caregiver in collaboration with other care providers to keep care as coordinated and consistent with the patient's goals.
  • Coordinates in-home diagnostics and therapies to manage patient symptoms.
  • Communicates with primary and specialty providers, informing them of the plan of care and negotiating treatments.
  • Performs documentation timely and accurately.
  • Performs all other duties as assigned.

Qualifications

  • Hospice and palliative certification (e.g. advanced certified hospice and palliative nurse from the Hospice and Palliative Credentialing Center.
  • Home care, hospice or palliative care experience.
  • Advanced care planning experience.
  • BCLS and/or ACLS certification.
