Responsibilities
- Conducts in-home and telehealth assessments for patients with complex needs.
- Prescribes new medications and performs medication reconciliation.
- Demonstrates sound clinical judgement and prescribing proficiency in all aspects of symptom management and end of life care.
- Facilitates family meetings, goals of care conversations, and ACP discussions.
- Leads oversight of care plans in conjunction with the rest of the clinical care team.
- Advocates for the patient and caregiver in collaboration with other care providers to keep care as coordinated and consistent with the patient's goals.
- Coordinates in-home diagnostics and therapies to manage patient symptoms.
- Communicates with primary and specialty providers, informing them of the plan of care and negotiating treatments.
- Performs documentation timely and accurately.
- Performs all other duties as assigned.
Qualifications
- Hospice and palliative certification (e.g. advanced certified hospice and palliative nurse from the Hospice and Palliative Credentialing Center.
- Home care, hospice or palliative care experience.
- Advanced care planning experience.
- BCLS and/or ACLS certification.
