Job Summary: The Care Manager-Registered Nurse (RN) Grant identifies and enrolls eligible participants into the Enhanced Care Management Program as part of the Projects for Assistance in Transition from Homelessness (PATH) Grant. Preforms a comprehensive assessment on participants and families, identifies their needs, and coordinates medical appointments, support groups, training, therapy sessions transportation and housing for them. Routinely meets with participants and family to provide education, support, identify growth and compliance within the program. Provides overall reports on services offered and compliance, as well as reports on individual participants growth and compliance submits reports as per PATH Grant requirements. Meets with community agencies, social workers, and other health care team members to ensure participants are getting the care that they need and are complaint with the recommended plan of care. Provides formal and informal education for community agencies, social workers, and the general public, and provides easy to understand informational pamphlets. Routinely meets with medical personal, healthcare teams social workers community agencies, reviews data and makes recommendations to build better healthcare, services and program for participants and education. Performs other duties as needed. Education and Experience: Bachelor of Science Degree in Nursing (BSN) required. Minimum three years of experience in Care Management, Primary Care, or home care nursing required. Experience working with older adults, families, and homeless individuals preferred. Knowledge and Skills: In depth knowledge of local community resources, and requirements to working under a federally funded grant. Exhibits strong assessment skills, problem solving skills, critical thinking skills, and able to analyze and interpret data. Able to: read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch. Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification issued by the American Heart Association required.
