Case Manager is a liaison for the criminal justice involved clients and treatment within Family and Children's Services. Case Manager will help the clients navigate the court system and break down barriers to getting to Mental Health and/or Substance Abuse Treatment. Works as advocate for consumers at risk of incarceration and as liaison between the agency, JusticeLink, and the court system under the direct supervision of the Program Supervisor. Functions within an interdisciplinary team that includes judges, court personnel, law enforcement, probation officers and other treatment providers. Responsible for communication flow between the court and agency staff regarding participant status and progress.

Responsible for attending policy meetings with members of the interdisciplinary team and ensuring that changes in policy are implemented and communicated. Case Manager develops and maintains positive working relationship with members of the interdisciplinary team. CCBHC is an integrated community behavioral health model of care intended to provide integrated, evidence-based, trauma-informed, recovery-oriented and person-and-family-centered care. The CCBHC offers the full array of CCBHC-required mental health, substance use disorder (SUD) and primary care screening services.

Qualifications:



  • Bachelor's Degree in the social sciences or related field preferred.
  • Case Management II certification "preferred" or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.
  • Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services.
  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues with respect for client rights and personal preferences in treatment are essential.
  • Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.
  • Strong organizational skills, superior oral and written communication skills, and excellent presentation/training ability.
  • Excellent program management skills and ability to conduct several projects simultaneously.
  • Experienced and skilled in coordinating and facilitating multidisciplinary meetings and possesses ability to interact across systems efficiently and effectively.

Family & Children's Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.

Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:



  1. Increased access to care and crisis services
  2. Expanded traditional community mental health and substance use services.
  3. Added Care Coordination and physical health screening for mental health clients
  4. Greater access to Social Services for clients' economic and social needs
  5. Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
