Position Overview

The Sr Clinical Care Navigator is responsible for providing care navigation support to assigned accounts utilizing products of Exact Sciences. Working cross-functionally throughout Exact Sciences, this role actively engages with health systems and patients to provide the highest level of customer service to ensure positive patient outcomes.

The Sr Clinical Care Navigator will be an exceptionally compassionate, creative, and motivated healthcare professional, who has a passion for serving patients, and a deep understanding of the healthcare ecosystem, from primary to specialty care. It is imperative that the Sr Clinical Care Navigator possess exemplary customer service skills, have a demonstrated ability to work independently, and function effectively as a member of the Exact Sciences team. The Sr Clinical Care Navigator will be required to collaborate with teams, such as Sales, Provider Support, Customer Care Centers, Medical Affairs, Logistics, and Analytics.

The Sr Clinical Care Navigator will need to understand, not only the technical and clinical aspects of the services that Exact Sciences offers but must also possess the ability to communicate professionally and effectively with all individuals, including external vendors and other internal Exact Sciences personnel.

The Sr Clinical Care Navigator's responsibilities include developing relationships with health care systems, vendors and patients, understanding and identifying various community resources, as well as developing and maintaining care navigation workflows. The Sr Clinical Care Navigator answers queries regarding care navigation as their primary responsibility. In this role, they will liaise with cross-functional internal and external teams to maintain a high level of customer service and ensure a positive customer experience.

This position is remote.

Essential Duties
Include, but are not limited to, the following:

  • Assist in implementing, co-creating, and updating care navigation workflows for health systems.

  • Identify and execute effective solutions to address inquiries.

  • Provide care navigation insights, assist in identifying community resources, and operational support to health care systems, as requested.

  • Advocate health system needs around care navigation with all Exact Sciences product teams.

  • Work through any customer or patient concerns with Customer Support Teams, i.e., CAR, CCC, Quality Improvement, etc.

  • Partner with Internal teams to develop efficiencies and process improvements to address patient and health system needs.

  • Strong customer service and communication skills; ability to incorporate customer and manager input.

  • Sound business problem-solving skills.

  • Ability to effectively build relationships.

  • Work on problems of diverse scope where analysis of data requires evaluation of identifiable factors.

  • Good judgment in selecting methods and techniques for obtaining solutions.

  • Ability to handle multiple projects at the same time but remain organized, with a strong sense of urgency for schedules.

  • Ability to receive little instruction on day-to-day work, general instructions on new assignments.

  • Support and comply with the company's Quality Management System policies and procedures.

  • Attend work regularly and reliably.

  • Ability to work nights and/or weekends, as needed.

  • Ability to work standing for approximately 50% of a typical working day. Ability to work seated for approximately 50% of a typical working day.

  • Ability to work on a mobile device, tablet, or in front of a computer screen and/or perform typing for approximately 90% of a typical working day.

  • Ability to work on a computer and phone simultaneously.

  • Ability to use a telephone through a headset.

  • Ability to comply with any applicable personal protective equipment requirements.

Minimum Qualifications

  • Bachelor's degree in Population Health or related field; or high school degree/general education diploma and 4 years of relevant experience in lieu of Bachelor's degree.

  • 2+ years of clinical experience working in health care system.

  • Proficient computer skills to include Internet navigation, Email usage, and word processing.

  • Proficient in Microsoft Office to include PowerPoint and Excel.

  • Demonstrated ability to perform the Essential Duties of the position with or without accommodation.

  • Authorization to work in the United States without sponsorship.

Preferred Qualifications

  • Experience in Quality, Population Health, Community Health, or Care Navigation within a health system.

  • Clinical degree or certification, i.e., RN, BSN, LPN, RMA, CMA, etc.

  • Proficient in the use of Microsoft Visio.

Salary Range:

$89,000.00 - $151,000.00

The annual base salary shown is for this position located in US - WI - Madison on a full-time basis. In addition, this position is bonus eligible, and is eligible to be considered for company stock at hire and on an annual basis.
