Job Summary

Under general direction from the Community Health Needs Assessment and Community Benefit Manager, this role will support and operationalize the community health needs assessment process on behalf of the Corewell Health System. This includes serving as the point person for insight, guidance, and expertise on the CHNA process including communicating data collection standards, community input best practice, prioritization of health needs, Implementation Strategy creation, and Implementation Strategy data collection on intervention progress. The Senior Community Health Needs Assessment Project Specialist will collaborate with tax and legal teams to ensure regional community health teams are navigating the CHNA process in a compliant manner and ensuring CHNA-related outputs and reports are shared with the appropriate stakeholders including Corewell Health tax, legal, hospital leadership, community health leadership, and community leaders. This role will also be responsible for ensuring governance-related operations are managed according to governance timelines. This role will be responsible, with other members of HEEAAT, to assess, evaluate, and iterate on past CHNA cycles in order to ensure CHNA deliverables provide the most value to Corewell Health and its partners. Lastly, this role will work closely with other members of the HEEAAT team on Community Health Needs Assessment (CHNA) activities, helping to ensure that community benefit activities and identified CHNA findings are in alignment.

Essential Functions
  • Lead and oversee conducting community health need assessments collaboratively with internal and external stakeholders across Corewell Health's service area of 21 counties
  • Establish and implement standardized best-practice workflows and toolkits for the CHNA process across Corewell Health
  • Collaborate with tax, legal and other internal teams to ensure CHNA activities are completed in an IRS compliant and timely manner.
  • Manages and coordinates several concurrent projects/investigations. Employs established project management techniques, tools and methodologies in managing projects.
  • Develop and present reports and recommendations based on CHNA findings
  • Advises on and coordinates data collection and data management processes, education of key resources in epidemiological methods and project management.
  • Continually assess the community benefit infrastructure in an effort to improve the processes and practices at Corewell Health.
  • Support the transformation of community benefits into a tool for advancing health equity and community health across the Corewell Health service area by creating and maintaining a community benefit communication plan for external stakeholders as well as internal partners such as governance and hospital leadership.
  • Contributes to the development and execution of data-informed initiatives to enhance and improve the health of targeted populations as identified in the Community Health Needs Assessment (CHNA) and other sources.
  • Leverages and supports community benefit expenditures and CHNA Implementation Strategies to align with the organization's strategic priorities.
  • Builds internal capacity and awareness among staff and leadership to address identified community health needs
  • Works with key internal hospital leaders to ensure inclusion of community benefits and community health needs within the organization's overall strategic and financial planning processes.
Qualifications

Required

  • Bachelor's Degree or equivalent
  • 5 years of relevant experience related experience
  • Related experience in community health data collection methodology, technical writing and continuous improvement methodologies and tools
