JOB PURPOSE:

The Lead Case Manager, under general direction of the Program Manager/Program Director provides staff support and mentoring of clients using a trauma-informed care, strengths-based approach in community living. Assist clients in developing/maintaining structure in their daily routines to achieve self-sufficiency.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:





  • Supervise Case Managers and interns to ensure all interactions align with the trauma-informed, strengths- based program design. Provide on-going program evaluation, staff training liaison for case managers, data tracking, random audits, and reporting to offer recommendations to the Program Manager to ensure continuous contract requirements and program development and quality are met.
  • Responsible for providing support to staff and clients related to day-to-day functioning of the program. Responsibilities are carried out by teaching, training, supporting, coaching, and acting as a check and balance.
  • Model appropriate Case Management techniques, specifically positive interactions, affirmative verbal guidance, effective communication and conflict resolution.
  • Assist with developing and maintaining partnerships with outside agencies to increase service potential for client success.
  • Support weekly staff meetings when Program Manager is not available; to assist in resolution of case-specific obstacles reducing client potential progress.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Master's degree in Social Work, Sociology, Psychology or related field. LPC, LCDC, or LMSW preferred.

EXPERIENCE: 5+ years of experience is a social service agency, 5+ years in a customer service focused environment. 2+ years management experience. Experience with mental health and/or a homeless population is a plus.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
