Under the direction of the Hospice Program Director/Senior Manager, the Mgr Clinical Services Hospice oversees the overall operations of the Interdisciplinary Team (IDT) and the provision of patient care. The Mgr Clinical Services Hospice ensures practice standards are followed and delivery of safe patient care.

Responsibilities

  • 1. Coordination and management of IDT services to patients and families by direct service staff.
  • 2. Manages the day-to-day operations, edits payroll, ensures adequate supplies and resources are available, and acts as the clinical resource.
  • 3. Performs duties in compliance with all applicable federal, state, and local laws and regulations.
  • 4. Conducts applicant interviews, participates in the hiring process with Human Resources, and assists with the orientation of new staff.
  • 5. Supervises and evaluates IDT staff, ensuring competency with practice guidelines and policies.
  • 6. Make joint visits as necessary, and may be required to make field visits to assist with volume.
  • 7. Ensures that IDT and other needed conferences occur and are documented by agency policy and Federal, State, and local regulations.
  • 8. Ensures accurate and timely scheduling and documentation.
  • 9. Works in collaboration with other disciplines and managers.
  • 10. Participates in staff meetings, in-service programs, and serves on committees as requested.
  • 11. Plays a significant role in the development, evaluation, and implementation of standards of practice and related policies and procedures. Interprets agency policies and Federal, State, and local regulations and insurance requirements of staff.
  • 12. Participates in Quality projects targeting clinical high-risk and problem areas for the Hospice Program.
  • 13. Participates in regulatory and financial audit activities to ensure compliance with Federal, State, and local regulations for the Hospice Program.
  • 14. Assists in the Quarterly Clinical Record Review process for the Hospice Program. Performs concurrent and discharged patient record reviews on a quarterly basis and as requested.
  • 15. Prepares periodic reports/dashboards as requested.
  • 16. May perform other duties as assigned.

Qualifications

EDUCATION

Registered Nurse with bachelor's degree in related field.

EXPERIENCE

Minimum of three years of full-time clinical experience in nursing with one year of home health/hospice experience. Previous supervisory responsibilities in the home care/hospice setting desirable.

LICENSURE

Registered Nurse with Valid (or eligible) Connecticut license required.

SPECIAL SKILLS

Excellent written and oral communication. Ability to effectively work with and motivate staff. Must be adaptable, flexible, have the ability to learn quickly, facilitate, use critical thinking, and manage people and projects. Strong computer skills.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, prolonged sitting, standing, and stairs.


Additional Information

Knowledge of hospice care, regulations, billing, quality, etc. preferred
Managerial experience preferred
