Interstitial Lung Disease Patient Population-

The primary role is to provide support for urgent clinic visits (sick visits and post-hospital follow-ups) for patients established with the ILD group. The majority of the urgent visits involve 1) assessing severity of illness (treat as outpatient, referral to ED or need for admission) in conjunction with the patient's provider 2) providing guidance for those being treated as an outpatient (obtaining RVPs in clinic, prescribing oral antibiotics, or supportive measures) 3) in conjunction with the ILD nurse, determining if O2 needs have changed and adjusting prescriptions accordingly. For post-hospital follow-up visits, the APP will review medications from hospital discharge, assess patient/caregiver understanding of any medication changes, and confirm that the patient is stable/improving from the time of discharge. The APP will be in the clinic with an MD provider who will serve as a resource for the APP. Two half-day sessions/week are needed. The temporary APP would be oriented by our current APP prior to starting.
