Job Title: Patient Navigator Specialist

Location(s): San Francisco, CA

Position Summary
The Patient Relations / Patient Advocacy Specialist is responsible for receiving, assessing, and resolving patient grievances and concerns in accordance with hospital policy, regulatory requirements, and patient rights standards. This role requires extensive interaction with diverse patient populations and close collaboration with clinical, administrative, and compliance teams to ensure timely and appropriate resolution of issues.

Key Responsibilities

  • Receive, investigate, document, and resolve patient grievances and complaints in a timely and professional manner.
  • Advocate for patients and families while ensuring compliance with hospital policies, Patient Rights & Responsibilities, and regulatory standards.
  • Serve as a liaison between patients, families, clinical staff, and hospital leadership.
  • Analyze grievance data to identify trends, risks, and opportunities for quality improvement.
  • Maintain accurate documentation and reporting in accordance with Joint Commission and CMS requirements.
  • Apply strong organizational knowledge to identify appropriate resources and escalate issues when necessary.
  • Work effectively with ethnically, culturally, and sexually diverse populations using trauma-informed and culturally competent approaches.
Required Qualifications
  • Direct experience handling patient grievances or working in patient advocacy within a hospital or healthcare system.
  • Minimum of three (3) years of relevant experience in healthcare, patient relations, public health, compliance, or equivalent hands-on experience.
  • Demonstrated experience working with ethnically, culturally, and sexually diverse populations.
  • Knowledge of HIV/STD treatment and prevention with an emphasis on harm reduction principles.
  • Strong working knowledge of:
    • Patient Rights & Responsibilities.
    • Joint Commission standards.
    • Centers for Medicare & Medicaid Services (CMS) regulations.
  • Proficiency in medical terminology.
  • Strong analytical skills, including data collection, compilation, and analysis.
  • Proven ability to assess grievances quickly and determine appropriate resolution pathways.
Education
  • Bachelor's degree in a related field and three (3) or more years of relevant experience OR
  • An equivalent combination of education, training, and hands-on experience
Preferred Certification
  • California HIV Test Counseling Certification.
