Position Title: Care Manager
Work Location: Remote OH (Ericka Cashero - North West - Lucas, Fulton, Wood, Ottawa)
Assignment Duration: 6 Months with possibility of extension and conversion
Work Schedule: Monday-Friday 8a-5p EST
Work Arrangement: Remote (Field-Based in OH - 50-60%)
Position Summary:
Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families. Everyone works together to ensure our members get the care and services they need to remain as independent as possible. Background & Context:
This is part of a larger hiring initiative of 30 staff.
Key Responsibilities:
Log on laptop top by 8am, review emails, tasks, and voicemails and respond to any urgent needs.
Review calendar for possible meetings and/or scheduled visits.
Staff typically arrange their days with the expectation of 5-7 visits required weekly.
Documentation must be completed within 24 hours.
Qualification & Experience:
Required: Master's Behavioral Health Professional
Required: LCSW, LMSW, LMFT, LMHC, LPC
Years of experience required: 2-4 years of related experience in case management or home health.
Discharge planning experience.
BH experience.
Computer Literate (knowledge of Microsoft)
Critical Thinker
Works well independently
Working Conditions & Physical Demands:
In field about 50-60% of the time
The rest of the time will be documentation from home
Occasionally (once a year) will need to travel to the Columbus The Organization location for team meets
