Job Title: Psychosocial Care Coordinator - Join Our Dynamic Healthcare Team!

Position Summary: Are you a dedicated professional ready to make a meaningful impact in patient care? We are seeking a skilled Psychosocial Care Coordinator to provide top-tier psychosocial support to our patients while collaborating closely with our esteemed medical team. In this vital role, you will conduct comprehensive assessments of patients' needs, inform them of their medical plan, and identify barriers to care. Your mission: to implement effective interventions and connect patients with valuable resources-ultimately enhancing their recovery journey within a compassionate and thriving healthcare environment.

Key Accountabilities:



  • Conduct Comprehensive Assessments: Identify and address the social and emotional needs of patients and their families to optimize medical treatment and outcomes.
  • Risk Assessment Expertise: Perform thorough suicide risk evaluations and provide appropriate safety recommendations to the medical team.
  • Facilitate Community Connections: Ensure seamless coordination of care by making essential referrals to multidisciplinary team members and community resources, alleviating psychosocial stress for patients and families.
  • Deliver Therapeutic Interventions: Utilize your expertise in crisis intervention, grief support, problem-solving, and psychoeducation to empower patients with healthy coping strategies.
  • Collaborate for Success: Work hand-in-hand with the medical team to create individualized care plans, identifying and addressing psychosocial barriers to safe discharge.
  • Maintain Accurate Documentation: Ensure timely and precise electronic psychosocial documentation and data tracking to support patient care initiatives.
  • Effective Communication: Engage in regular communication with team members regarding patient needs and interventions, actively participating in team meetings and rounds to foster a collaborative environment.


Qualifications:



  • Master's Degree in Social Work (MSW) required.
  • Successful completion of JHACH's intern program or a comparable internship in a healthcare setting.
  • Outstanding listening and oral/written communication skills.
  • Proficiency in effective documentation within the electronic medical record system.

Salary Range: Minimum 27.85/hour - Maximum 44.57/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
