The Salvation Army Midland Division Shelter located in Jefferson City, Missouri, is looking for a Pathway of Hope Case Manager and Emergency Assistant Case Manager! This individual will Manages client cases in the Center of Hope Emergency Shelter and Emergency Service Assistance. Position must perform duties in compliance with all active funding requirements and may be involved in reporting to grants funders.

This is a full-time, 40 hour a week position. Non-exempt $21.00 per hour.

Essential Functions

Center of Hope:



  • Conduct initial interviews with all Emergency Shelter clients to determine immediate needs.
  • Develop client goal plan and meet periodically with clients on caseload to update goal plans and client activity notes in case files, encouraging clients to access community resources for additional assistance.
  • Perform case manager duties in accordance with the terms of all active grants.
  • Make referrals to appropriate services and agencies for material, medical, and other types of assistance.
  • Refer client issues to case conference for resolution. Assist with resolution of client issues and problematic case management as directed in the absence of the Shelter Director.
  • Complete documentation as mandated by contract, policy and procedure, and/or Salvation Army practice in the approved computerized reporting system.
  • Maintain and report statistic to the administration office. Enter client data on a timely basis into HMIS and ensure data quality.
  • Conduct the initial intake assessment of applicants to the Pathway of Hope program to determine eligibility.
  • Develop, implement, and monitor family/individual case management plans, including short and long term goals, designed to help people achieve the highest level of self-sufficient with the greatest expediency. Document all interactions in case file notes.
  • Provide case management services for families/individuals as they implement the case management plans. In the office, regularly meet with family/individuals in the program. Periodically inspect Pathway of Hope housing off site.
  • Provide information and referral to clients on issues such as education, employment, housing, utilities, budgeting, etc. to improve client functioning and eliminate barriers to increased assets and permanent housing.
  • Network with other agencies and organizations within the community to maximize services to our clients.
  • Participate in staff meetings as directed.
  • Maintain confidentiality, proper client staff boundaries and comply with Safe From Harm guidelines.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters good relationships with clients, co-workers, and supervisors. This includes but is not limited to such actions as: resolution of conflicts in a professional manner; courteous treatment of staff, visitors, and clients; respect of others' property and person; and professional and appropriate communication to and about clients, co-workers and supervisors.



Education and Experience

Requires a bachelor's degree in social work, psychology, or other related field, accompanied by at least one year of work experience in human service delivery, administration, or management. Combination of education and experience may be considered. Ability to work with diverse and challenging personalities is essential. Must demonstrate good verbal and written communication skills and capable of enter and retrieve data from a computer. Proficient with Microsoft Office WORD, EXCEL, and email applications. Ability to relate to people in a courteous and professional manner/good interpersonal skill. Requires good organizational skills with the ability to handle multiple tasks at the same time. Must be a team player, a self-starter and perform well with minimum supervision and demonstrate ability to think independently and exercise sound judgment. Requires a high level of confidentiality. Maintain and exhibit a willingness to assist in carrying out The Salvation Army mission. Must participate in HMIS training and be certified for data entry into HMIS within 60 days of employment, preferably within 30 days. Willingness to continue educational skills as necessary for job performance.

Physical and Working Conditions

Must be capable of performing the above essential duties in an office environment to include using a computer, answering phones. Most work will be indoors in a temperature-controlled environment.
