The Patient Navigator is a member of the inter-connected services established through the Massachusetts Department of Public Health/Bureau of Substance Addiction Services Hospital Based Substance Use Services Grant to improve access to and reduce utilization of acute services.This position supports patients with identified substance use disorders by connecting to the healthcare system and receiving timely care through the establishment of a Medication Assisted Treatment Bridge Clinic. The Navigator is responsible for engaging patients with substance use disorder services within the UMass Memorial-Harrington Healthcare system and external agencies. The Navigator serves as a resource and liaison between patients, practitioners, clinical and administrative staff. The Navigator will provide short-term case management with patients to identify their barriers to healthcare and connect them to the resources they may need including, but not limited to, state financial assistance, insurance, counseling, and/or transportation. The Navigator is vital in providing proactive attention to patients to optimize and secure connections to on-going treatment by promoting the continuum of care. The Navigator expedites services and educates patients on the services provided by UMass Harrington Addiction Immediate Care Bridge Clinic or making external referrals when necessary. Major Responsibilities: * Identifying patient eligible for grant criteria via access through the Emergency Department * Data collection and Entry * Building community relationships with outside referral sources * Providing reliable transportation for patients as needed * Providing case management and referrals to meet patient needs * Connecting patients to the UMass Harrington Healthcare Behavioral Health continuum of care * Providing follow up contact with patients while part of the Bridge Clinic Standard Staffing Level Responsibilities: 1. Complies with established departmental policies, procedures, and objectives. 2. Complies with all health and safety regulations and requirements. 3. Maintains regular, reliable, and predictable attendance. 5. Performs other similar and related duties as required or directed. All responsibilities are essential job functions. Position Qualifications: 1. Experience with substance use disorders and mental health disorders preferred or willingness to learn. 2. Excellent communication skills and interpersonal skills. 3. Attendance to various staff meetings in accordance with grant requirements and to ensure program staff involvement. 4. Valid driver's license and proof of insurance 5. Strong organizational skills 6. Willingness to work with a multi-disciplinary team. License/Certification/Education: Required: Bachelor's Degree Preferred: Certified Alcohol and Drug Counselor (CADC) or Licensed Drug and Alcohol Counselor (LADC) preferred Experience/Skills: Required: Experience with substance use disorders and mental health disorders preferred or willingness to learn.
