Description The Social Worker, LICSW will function as a core member of a multidisciplinary team and communicate patient's health care needs with primary care, behavioral health services, health care facilities and/or payers. Acts as an advocate and helps patient gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. The position takes responsibility for management of a therapeutic caseload (individual and family therapy to children, adolescents, and adults as requested) providing timely charting and record keeping. The Social Worker may provide evidence-based treatments or work with other mental health providers when such treatment is indicated. Providence caregivers are not simply valued - they're invaluable. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
