Care Coordinator - Birth to Three Program

Full-Time | Hybrid | Waukesha, WI

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a full-time Care Coordinator to join our Birth to Three Program, a federally mandated Early Intervention program supporting children ages 0-3 with developmental delays or disabilities and their families.

This role focuses on strengthening families, enhancing child development, and coordinating services using a family-centered, trauma-informed approach. After training, this position offers remote flexibility when not conducting home visits. The team meets every other Tuesday at the Waukesha office. This is a benefit-eligible, full-time position.


What You'll Do

As a Care Coordinator, you will partner closely with families, service providers, and community resources to support young children's development.

Key responsibilities include:



  • Conducting functional assessments and reassessments
  • Developing and implementing individualized, family-centered service plans
  • Coordinating and facilitating services aligned with care plans
  • Supporting families through education, advocacy, and collaboration
  • Maintaining accurate and timely documentation in compliance with regulations
  • Coordinating and facilitating family-centered team meetings
  • Collaborating with internal teams, supervisors, and community providers
  • Managing a flexible schedule based on client needs, including occasional evenings or weekends
  • Meeting productivity and service delivery expectations


Qualifications

Education & Experience



  • Bachelor's degree in Social Work, Child Development, or related human services field required
  • One year of supervised experience working with families with special needs preferred

Additional Requirements



  • Valid driver's license and reliable transportation
  • Ability to meet LSS driving and insurance requirements
  • Strong documentation, communication, and organizational skills
  • Comfortable working in home, office, and community-based settings
  • Ability to respond appropriately in crisis situations


Work Environment

  • Community-based role with daily travel
  • Moderate physical activity, including bending, kneeling, stair climbing
  • Travel may include occasional overnight stays
  • Exposure to various environments while serving families in their homes and communities
