HonorHealth is looking for a Produce Prescription Program Manager for a new program that prescribes vegetables to patients with pre-existing conditions/concerns as an intervention for those on Medicare or Medicaid. This prescription would be a redemption certificate or voucher issued to see improvement in health, educationto the patient, and to see outcomes vs. participation n the program. The mobile farm is a 6-month period for improvement.

The ideal candidate must meet the minimum requirements of bachelor's degree in pubic health, community health, social services, health administration or a related field or equivalent experience. In addition, a minimum of 2 - 3 years experience in community health, patient navigation, outreach coordination or similarroles with experience in food as medicine, SDOH intervention or grant funded project. To ensure success in the role an attention to detail, being organized, passionate & outgoing, and the ability to be flexible as this is a hybrid role and will be on campus at Desert Mission, Medical Group offices, and the farm stand. A good communicator and someone whose innovative and a good collaborator.

Responsibilities

Job SummaryThe Program Manager serves as the central coordination and communication point for HonorHealth Desert Mission's Produce Prescription (ProduceRx) Program. The role supports patients referred by clinical teams by confirming eligibility, guiding them on how to access and redeem their Produce Prescription benefits, and ensuring timely connection to Desert Mission for services. The Program Manager works with Medical Group leaders, clinic staff,Integrative Medicine, Desert Mission, and the research team to maintain a reliable referral, eligibility, consent, redemption workflow, support the developing patient identification process, and meet research and evaluation requirements. Essential FunctionsReferral Intake, Eligibility, and Informed Consent
  • Receive participant referrals from participating clinics.
  • Confirm eligibility using established ProduceRx criteria.
  • Explain program expectations in plain language and ensure informed consent is documented according to IRB requirements.
  • Maintain accurate logs of referral, eligibility, and consent status.
Patient Guidance and Navigation
  • Provide clear instructions to participants on redeeming ProduceRx benefits.
  • Offer simple materials and reminders to maintain engagement and support followthrough.
Participant Flow Coordination with Desert Mission
  • Transmit eligibility and consent information to Desert Mission promptly to enable timely activation of services.
  • Support tracking of participant engagement and redemption milestones.
Collaboration With Medical Group Operations
  • Partner with clinic staff to maintain consistent referral workflows.
  • Share updates on referral patterns, operational issues, and participant flow, within scope.
  • Support continuous improvement of the clinic identification process.
Research Coordination Support
  • Collaborate with research staff to meet data collection, documentation, and IRB protocol requirements.
  • Maintain auditready program records to support grant and evaluation deliverables.
  • Performs other duties as assigned.
