QUALIFICATIONS:



  • High school diploma or GED required; Minimum of one year of related experience may be substituted for degree requirements
  • Experience working with homeless and/or at-risk populations preferred.
  • Strong oral and written communication skills.
  • Ability to communicate and work effectively with a diverse group of residents, staff, and community partners.
  • A demonstrated ability to work independently with limited supervision
  • Strong troubleshooting and problem-solving skills
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint, Publisher), email, and internet applications.
  • Ability to prioritize tasks and manage time effectively
  • Must be available to work occasional evenings, weekends, or on-call shifts as needed
  • Ability to pass a background check
  • Valid driver's license with an acceptable driving record


RESPONSIBILITIES:



  • Oversee compliance with all facility rules.
  • Provide support to all residents in the Vocational Services program.
  • Assist with facilitating the intake and discharge process and client orientation.
  • Complete accurate and timely data entry into the HMIS system
  • Coordinate care with Vocational case management team and others on behalf of program participants for eligibility documentation, mental and physical health services.
  • Maintain, record, and report the progression of program participants to the Vocational Case Management team.
  • Attend all scheduled team meetings and training.
  • Other duties and special projects as assigned.

The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus. Must have the ability to operate telephone, a desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
