      Position Summary:
      The Wellness Nurse provides nursing care and supervision to residents in the community. This includes direct resident care, conferring with other disciplines, referring to community resources and/or utilizing resource materials as needed. This position participates in care conferences and staff meetings, completes paperwork in a timely manner, and communicates with health care providers, Eskaton support staff, and other community persons as needed or directed. The Wellness Nurse is also responsible for conducting, coordinating, and tracking training for community care staff.

      Essential Job Functions:

      Comprehensive Nursing Care:



      • Promotes the success of others.
      • Demonstratesappropriate physicalassessment skills forinitialand follow-up appraisal of resident status.
      • Possesses an adequate knowledge of clinical disease entities.
      • Demonstrates ability to perform nursing treatments and procedures as prescribed for assigned residents.Performs those treatments in an appropriate manner.
      • Demonstrates adequate knowledge of specialized diets and medications commonly prescribed to residents andassistswith administration and management as needed.
      • Demonstrates knowledge and awareness of rehabilitative and preventative aspects of care and ensures resident safety, comfort, and protection.
      • Supervises and directs care given to residentsby Resident Care Associatesand Resident Medication Assistantsin an appropriate manner.Ensures continuity of care by appropriately directing, coaching, and counseling the support staff.
      • Participates in the training of all care staff.
      • Assistsphysicians or physician extenders in making rounds and assessments of residents (if applicable).
      • Marketing tocommunityand assisting with tours as needed.


      Documentation:



      • Documentsresidentstatus changes, resident instruction, resident/family response, and specific plan for ongoing care.
      • Maintainswritten care plans reflecting resident status ina timelyandaccuratemanner.
      • Completespre-placement and routine functional evaluations/assessments per Eskaton policy.
      • Followsdocumentation procedures as required by RCFE regulations.Usesaccuratemedical terminology.
      • Maintainsall records related to staff education and training in the community.


      Personal and Professional Development:



      • Attendsworkshops, seminars, and classes to further develop professional knowledge/skills byseekingor accepting new and challenging assignments whenrequested.
      • Conductsshadowing requirements for new Resident Medication Assistants.
      • Participatesin orientation programs for new employees, students, or selected individualsdesignatedby the community.
      • Participateson internal committees/projects when requested andparticipatesin staff meetings on a regular basis.


      Care Management:



      • Communicatesinformationregardingresidents at team meetings, and coordinates care provided to the residents.
      • Establishesshort term andlong-termgoals for total resident care and management.

      Starting pay ranges from $27.90 - $39.03 per hour. Factors such as scope, responsibilities, experience, education, job-related skills, internal equity, and market considerations may influence base pay.

      Qualifications:

      Education:



      • Valid California Licensed Vocational Nurseor Registered Nurse license.
      • Current CPR certification.







                    Training and Experience:



                    • Minimumtwoyearsrecent skilledor assistedlivingnursingexperience preferred.
                    • Prior supervisory experience.


                    Knowledge and Abilities:



                    • Possesses an in-depth knowledge of comprehensive nursing care whichincludesclinical disease entities, the physical assessment skillsrequiredforinitialand follow-up appraisal of all assigned residents, resident treatments, specialized diets, and medication regime for individuals.
                    • Must also be aware of the rehabilitative and preventative aspects concerning each resident, their safety, comfort, and protection, the usual performances indicated for disease prevention and restorative measures, the ability to document professional nursing services provided with a realistic plan for reaching short and long term goals, and to be able to make decisions regarding safety needs of residents including adequate equipment.

                    Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
