Community Support Specialist Diamond Grove Center is searching for a Community Support Specialist at their MYPAC program located in Starkville, Miss to jour our team at Diamond Grove Center. This position will be PRN(as needed). Diamond Grove Center Hospital is a premiere behavioral hospital that has been providing compassionate care to children and adolescents since 1997. Diamond Grove Center is located in the beautiful city of Louisville, Mississippi. Diamond Grove Center is a 85 bed, psychiatric behavioral health hospital that offers inpatient acute care as well as residential treatment for adolescents and intensive outpatient programs. The Community Support Specialist is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and youth and family centered. Services are primarily community-based, and participating individuals have mental health treatment needs and are in need of support additional to outpatient therapy services. The Community Support Specialist handles new referrals, creates service plans, reviews progress and makes determinations about closure and transitions to lower levels of care. The Community Support Specialist coordinates the flow of clinical information across agencies and providers while protecting the confidentiality of individuals served. The Community Support Specialist builds partnerships with enrolled individuals with the goal of helping them achieve wellness and autonomy.Must be available to work a flexible schedule to accommodate occasional evening and weekend hours to meet job responsibilities.Must be at least 21 years of age, have a valid Mississippi driver license, a good driving record, proof of legally appropriate auto liability insurance, and must be willing to use personal vehicle for agency business.
