Job Description

The Center for Child Well-Being, housed within the Department of Psychiatry, in collaboration with the Maternal-Fetal Care Center in the Department of Obstetrics, Gynecology, and Women's Health, seeks two full-time Cardiometabolic Resource and Referral Specialists.

This full-time, grant-funded, exempt, on-site position supports pregnant and postpartum patients and the professionals who care for them by connecting callers with timely, accurate, and appropriate cardiometabolic and social-service resources throughout Missouri.

Required credential

Current Missouri licensure as a registered nurse (RN) or licensed practical nurse (LPN).

About the Maternal-Fetal Care Center (Department of Obstetrics, Gynecology, and Women's Health)

The Maternal-Fetal Care Center provides comprehensive, multidisciplinary care for women with high-risk pregnancies through an academic practice committed to excellence in patient care, education, research, and innovation. The Center brings together Maternal-Fetal Medicine specialists and a broad network of multidisciplinary partners, including General Obstetrics & Gynecology, Neonatology, Pediatric Subspecialties, Genetics, Cardiology, Anesthesiology, and other specialists, to provide coordinated care for complex maternal and fetal conditions. The Center's comprehensive services include outpatient and inpatient maternal-fetal medicine consultation, a robust ultrasound program with an AIUM-accredited Ultrasound Unit and echo-certified sonographers, comprehensive genetics services, and specialized multidisciplinary programs such as the Cardio-Obstetrics. As an integral part of the University of Missouri School of Medicine, the Center also provides an outstanding educational environment for medical students, OB/GYN residents, and Maternal-Fetal Medicine fellows while advancing clinical research and innovation in high-risk obstetric care.

Key responsibilities

  • Respond to resource and referral requests received by telephone, email, or other program channels and complete a focused assessment of each caller's needs.
  • Performs all activities in compliance with HIPAA, health and safety regulations, and organizational policies and procedures.
  • Identify and connect patients, families, and healthcare professionals with cardiometabolic, maternal-health, and community resources, including services related to hypertension, diabetes, nutrition, medication access, transportation, insurance, behavioral or mental health, and social needs. Conducts follow-up with patients and families as necessary and indicated.
  • Provides responsive, respectful, and service-oriented support, fostering engagement and satisfaction.
  • Provide warm handoffs, follow-up, and problem-solving support for complex or difficult-to-place referrals while maintaining appropriate role boundaries.
  • Develop and maintain accurate statewide resource information, with particular attention to availability, eligibility, waitlists, and access barriers in rural Missouri communities.
  • Coordinate closely with all appropriate members of the Maternal Health Access Project team, including but not limited to MHAP Cardiometabolic Program Lead, Nurse Health Educator, clinical consultants, Resource and Referral team. Develop strong relationships with external partners.
  • Document contacts, referrals, outcomes, and follow-up activities in program data systems and support reporting, quality-improvement, and evaluation activities.
  • Participate in outreach, education, team meetings, and continuous improvement of referral workflows and resource tools.
  • Protect confidential information and follow university, program, and applicable regulatory requirements.
  • Perform other duties as assigned.
Shift

Primarily 8:00 a.m.-5:00 p.m., Monday-Friday.

This position requires some after-hours and weekend shifts to provide full coverage for our resource access hotline. Hours may shift on a weekly basis.

Minimum Qualifications

Master's degree in psychology, counseling, marriage and family therapy, or social work and relevant licensure (or eligible to be licensed within 12 months) within the State of Missouri.

Preferred Qualifications

Education and experience: Experience in cardiometabolic and/or maternal health, particularly hypertension and diabetes management. Three years of related work experience is preferred.

Credentials: Current Missouri RN or LPN license.

Knowledge and skills: Familiarity with Missouri health and social-service systems, especially rural resources; experience with resource navigation, care coordination, telephone-based support, electronic documentation, and interdisciplinary teamwork; strong communication, organization, cultural responsiveness, and problem-solving skills.

Anticipated Hiring Range

Salary Range: $60,000 - $65,000 annually
Grade: GGS - 010
University Title: Senior Counselor, Health Welfare Wellness
