Optum is seeking a Behavioral Health Advocate to provide field-based case management and care coordination services for adult Medicaid members throughout Manhattan. This role focuses on supporting members' behavioral health needs, reducing hospitalizations, facilitating successful transitions of care, and connecting individuals with community-based resources. The BHA serves as a key liaison between health plan teams, providers, treatment facilities, and community organizations. Position Highlights Field-based behavioral health case management for adult Medicaid members Coordinates aftercare planning and transition-of-care services Connects members with behavioral health, medical, and community resources Partners with hospitals, treatment teams, and community providers Focus on reducing readmissions and improving community stability Required Qualifications Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage & Family Therapy, or related behavioral health field Active New York independent license (LCSW, LMHC, LMFT, or Licensed Psychologist) 2+ years of post-licensure experience in a mental health setting Proficiency with Microsoft Office (Word, Outlook, Excel, and Teams) Residence in or near Manhattan with reliable transportation for community-based visits
