Job Summary

The Transplant Nurse Practitioner is the primary nursing resource and the clinical expert in the care of the inpatient liver and kidney transplant recipient as well as the in-patient dialysis access patient. The NP has primary responsibility for the day-to-day clinical management of inpatients on the Transplant Surgery Service. This responsibility involves coordinating the delivery of interdisciplinary medical and surgical care under the guidance of the Transplant Surgery attending physician. The Nurse Practitioner also functions as the liaison between the Transplant Surgery attending physician and the house staff, consulting ancillary providers, hospital departments, patient, family, community physicians, and insurance companies. The NP monitors transplant organ function and assesses pre and post operative patient condition for potential issues and/or complications. The NP actively coordinates provision of multi-disciplinary care within UCDHS as well as coordinating care provided by a variety of professionals within other health care systems. The NP coordinates the discharge plan in conjunction with the unit discharge planner, the transplant pharmacist and outpatient transplant coordinator. The NP provides ongoing patient and family education and assesses appropriate medical intervention for effective resource management and quality assurance. The NP focuses on holistic care of the individual patient within their support system. #CA-AP

Minimum Qualifications

  • Graduation from an accredited acute care nurse practitioner program (ACNP)
  • Possession of a valid California license as a registered nurse
  • Possession of a valid Nurse Practitioner license
  • Possession of a valid DEA number and furnishing certificate
  • Possession of valid NPI number
  • Possession of current CPR certification
  • Graduation from an accredited school of nursing
  • Master's degree or equivalent combination of education and experience.
  • Ability to read, write, speak, and understand the English language and utilize medical terminology appropriately.
  • Knowledge and skills related to transplantation including care of the pre-op and post-op transplant recipient care of the donor nephrectomy patient care of the dialysis access patient
  • immunology and HLA testing and its implications
  • Ability to independently set priorities, make. clinical decisions and organize work in a high paced environment.
  • Ability to work accurately and quickly under pressure.
  • Knowledge of medications, indications, dosage ranges, side effects, and potential toxicity.
  • Ability to stand for extended periods while making rounds and evaluating patients. Able to work for 4 hours without taking a break.
  • Skills to perform CPR.
  • Ability to assess educational needs of patients and provide education appropriate to the individual learning style and learning age.
  • Knowledge of legal implications for clinical practice.
  • Knowledge of performance improvement and research process
  • Ability to develop policies and procedures to meet program needs
