Schedule: Monday - Friday, 8am - 4:30pm
Status: Full-time, Non-Exempt (Hourly), Non-Management

SUMMARY

The People Concern is partnered with the Department of Health Services (DHS) and their Housing for Health (HFH) Program to provide housing and Intensive Case Management Services (ICMS) to persons experiencing homelessness. The Project-based Housing Program houses individuals transitioning from homelessness in subsidized housing units that are attached to supportive services.

Permanent Supportive Housing clients are linked to ICMS case managers who provide home-based and field-based interventions clinical services while employing trauma-informed care and harm reduction approaches. Clients in our programs are a diverse group with complex trauma experiences complicated by medical, mental health and substance use barriers. Clients remain in the program for as long as they require subsidized housing. For many individuals this may mean their entire lifetime.

This Coordinator role is responsible for the screening, interviewing, onboarding, and training housing case management staff for placement with Permanent Supportive Housing ICMS teams. The Coordinator serves as a liaison between internal Recruitment and Housing department staff to ensure collaboration around thorough onboarding of new case management staff while upholding the agency's and funder staffing standards. They also build collaborative working relationships with site-based teams to support successful integration of new staff.

ESSENTIAL DUTIES & RESPONSIBILITIES



  1. Alongside Program Manager of Housing Onboarding, conduct ongoing assessment of Housing Department staffing needs.
  2. Screen resumes of candidates for contract eligibility and conduct phone screens to further assess goodness of fit with existing teams.
  3. Schedule interviews in collaboration with team-based case management leadership.
  4. Check references to assess for staff employment history and experience.
  5. Work with The People Concern Human Resources and Recruitment staff to complete all necessary personnel action forms are completed to prepare for onboarding and equipment connection.
  6. Support multi-week training program implemented with new Housing Department to Case Managers.
  7. Ensure training staff utilizing DHS' "Whatever It Takes" approach to client engagement.
  8. Reinforce evidence-based practices throughout training and onboarding including Housing First, Harm Reduction, and Trauma-informed Care.
  9. Provide administrative support to new case management staff during their initial onboarding phases at The People Concern.
  10. Ensure new staff abide by all agency and department policies and procedures.
  11. Travel to multiple sites throughout each day and/or week to provide new case management staff orientation to housing settings and facilitate skills shadow opportunities.
  12. Support new case management staff in coverage duties until permanent assignment to housing ICMS team.
  13. Provide client coverage and intake support when recruitment and onboarding needs fluctuate.
  14. Ensure all documentation, charting, and data collection for program clients is complete, timely, and accurate through on-going audits of electronic databases and chart reviews.
  15. Other Housing Department duties, as assigned.



QUALIFICATIONS



  1. Bachelor's degree, preferred or a minimum of two years of full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions.
  2. Experience facilitating training, preferred.
  3. Minimum of one year hiring and staff management experience, preferred.
  4. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.



  1. Current, valid Driver's with an acceptable driving record.



WORK ENVIRONMENT



  1. Field (may need to travel) and indoor office environment.
  2. On occasion walk or drive to different local sites.
  3. Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.
  4. Will necessitate working in busy and loud environments.
  5. Will be exposed to elements like cold, heat, dust, noise and odor.
  6. May need to bend, stoop, twist, and sit throughout the day.
